Clarity Through Community believes that lasting change happens through collaboration to create a more connected system of care for individuals experiencing behavioral health and substance use challenges, no matter where they're located in the U.S. Our partnership program brings together hospitals, healthcare providers, law enforcement agencies, and community organizations to create a more connected system of care for individuals experiencing behavioral health and substance use challenges.

As a member, you gain more than access to Intensive Care Facilitation services. You gain a trusted extension of your team that remains engaged before treatment begins, throughout the treatment process, during discharge, and for at least 90 days after treatment ends. Every individual served also receives ongoing access to the Sayin platform, ensuring they remain connected to local resources and support long after their immediate crisis has passed.

Care Connection

Best for: Individuals and Families seeking care for themselves or a loved one

When someone you love needs behavioral health or substance use treatment, navigating the system can feel overwhelming.

Available nationwide, Care Connection pairs every individual with a dedicated Care Facilitator who remains alongside them from the initial assessment through treatment, discharge, and continued recovery support.

Families receive guidance, advocacy, and ongoing access to community resources through the Sayin platform, helping ensure that support continues long after treatment ends.

Provider Care Connect

Best for: Independent Healthcare Providers

Provider Connect gives therapists, physicians, psychiatrists, nurse practitioners, counselors, and other licensed professionals direct access to Clarity's Intensive Care Facilitation services.

    We have partnered with Sayin, a healthcare resource platform, to enable us to more quickly and efficiently recommend high quality resources and care to our clients. We have many affiliated partners that we refer to more often but we refer primarily based on the best clinical fit for each individual.
