The Reset Project

The Reset Project is a comprehensive reentry and transition program designed to support both

previously incarcerated individuals and transitioning veterans, particularly those experiencing

mental health and substance use challenges. Grounded in the principle of continuum care, the

program ensures that participants receive seamless, coordinated support before, during, and after

critical life transitions—whether returning to civilian life after military service or reintegrating

following incarceration. Through intensive individualized care facilitation, participants are

guided through a structured yet flexible pathway that addresses behavioral health, social

determinants of health, and long-term stability. Research highlights that individuals transitioning

from structured systems, such as the military or correctional facilities, often face significant

barriers including unmet mental health needs, difficulty accessing care, unemployment, housing

instability, and limited social support. To help improve outcomes and reduce barriers to

successful reintegration, The Reset Project follows participants for a minimum of 90 days,

providing ongoing support, treatment coordination, resource navigation, and accountability to

promote long-term stability, recovery, and community connection.
